Instructions for downloading and filling in forms:
Click the “Download Form” button for the specific form you wish to download, fill out and submit.
Save the form on your computer.
Open the form you have downloaded using Adobe Acrobat Reader. If you do not have Adobe Acrobat Reader X Standard, or later, available on your computer you can install it by clicking here. This action will take you to the Adobe Acrobat Reader installation page. Uncheck the optional offers, click “Install Now” and follow the instructions.
Fill the form fields as necessary. You can save a partially filled form and complete it later.
Click the “SUBMIT” button on the last page at the lower right side.
The “SUBMIT” action will check the form for completeness. If required fields are not filled in, or if the format of fields is not correct, you will be prompted to revisit those fields and take corrective actions. After making changes, click the “SUBMIT” button again for form verification.
Once the form check has completed and passed, the field shading is removed, fields are locked from further changes and Signature date fields are filled with the current date.
You will be prompted to save a copy of the filled out form on your computer with an advisement to give it a different file name than the unfilled form file.
You will be prompted to print copies of the filled out form. Sign one of these copies and return it to Little Stars by mail or in person.
